Interview questions and notes for an interview with opera singer Shirley Verrett, written by Lisa Hoffman, consisting of one sheet of white paper with typewritten interview questions on side one. Both sides contain numerous handwritten notes in blue ink, pink marker, and graphite. There is a noticeable seam down the middle of the paper from previous folding, and a small loss at one corner.
