Around the outer edge of the medal are the words [ATLETICA COPPA DEL MONDO / III I.A.A.F. WORLD CUP / ROMA '81]. The back of the medal has no images or writing. The medal is textured, with a smooth, slightly inset rectangle in the center. The medal is attached to a red, white, and green striped ribbon at its top.
